Error & Incident reports: what they are and why we use them

You may or may not know about or receive Error & Incident reports, but here is a refresher of what they are and why we use them in ASR. Error & Incident reports are used to share information in order to help troubleshoot issues and clarify the status of errors and incidents, usually regarding system issues. There is also historical value in the reports; they help ensure we have made corrections and improvements to avoid the same errors in the future.

Complete an Error & Incident form when a system issue or error is discovered and the issue may impact multiple staff, students, or teams. Once completed, the report is sent to a distribution list that includes many teams within ASR as well as some contacts outside of ASR such as system campus partners.

The form can be found in the right side bar “Quick Links” section of the ASR intranet. If you are unsure you should submit a report, have a conversation with your supervisor or team.
